Our expert staff is more than prepared to offer comprehensive medication management services to our patients.

Benefits of Medication Management

While you or a loved one is in the middle of recovery, it can be easy to miss medication that is helpful on the road to wellness. There are many benefits to having medication management services, including:

Medication errors are a common cause of hospital admissions and can lead to serious harm or death. Our medication management services can help prevent these errors by ensuring that patients receive the right medications in the right doses at the right times.

Medications are an important part of mental health treatment, but they can only be effective if they are taken correctly. Our medication management services help ensure that patients are taking medications as prescribed and on schedule, which can improve treatment outcomes.

Medications can have side effects, and some patients may experience more side effects than others. Our medication management services can help minimize side effects by carefully monitoring medication use and making any necessary adjustments to medication doses or regimens.

Many patients have questions or concerns about their medications. Our medication management services provide patients and caregivers with education and resources to help them understand the purpose, dosage, and side effects of each medication.

Patients with mental health disorders may receive care from multiple healthcare providers. Prescription medication management at Acera Health helps ensure that all healthcare providers are aware of all medications being taken, which can reduce the risk of prescription medication interactions and other complications.

Medication Management at Acera Health

Our patients can access our medication management program by scheduling an appointment with one of our healthcare providers on staff. During the appointment, the healthcare provider will review the patient’s prescription medication list, assess medication effectiveness and side effects, and make any necessary adjustments to medication doses or regimens.

Patients, family members, and caregivers should be prepared to discuss any concerns or questions about medications and should bring their medication list with them to the appointment.

How We Handle Medication Management

At Acera Health, we are committed to putting our patients first. We believe that medication management is an important part of a mental health treatment plan, and we are dedicated to providing the highest quality medication management services to our patients.

We work closely with our patients and their caregivers to develop personalized medication management plans that meet their unique needs. We are always here to listen, educate, monitor, communicate, and adjust as needed to ensure that our patients receive the best possible care.

Our Medication Management Process

1. Assessment

During the initial assessment, our healthcare providers will review the patient's medical history, medication list, and any other relevant information. They will assess the patient's mental health and medication needs to develop a personalized medication management plan.

2. Medication Review and Reconciliation

Our healthcare providers will review the patient's medication list and reconcile any discrepancies or potential interactions. This step is crucial to ensure that patients are not taking duplicate or conflicting medications.

3. Monitoring

We closely monitor our patients' medication use, side effects, and effectiveness. We may adjust medication doses or regimens as needed to ensure that patients are receiving the best possible care.

4. Education and Support

We believe that education and support are key to effective medication management. We provide our patients and their caregivers with information and resources to help them understand their medications and how to take them properly. We also offer ongoing support to address any concerns or questions that may arise.

5. Coordination of Care

Medication management is an important part of mental health services, but it's not the only part. We work closely with our patients' other healthcare providers to ensure that everyone is on the same page when it comes to medication management and overall treatment.

6. Regular Follow-Up

We believe that regular follow-up is crucial to ensuring that medication management plans are effective. We schedule regular appointments with our patients to monitor their medication use, side effects, and overall health.
